Collection from SHC Sources

We import personal information from SHC systems and sources. The information includes:

  • First and last name

  • Phone number.

We collect the following personal information when you inquire to apply for a position:

  • First, middle initial and last name

  • Email address

  • Phone number

  • Position applying for

  • Preferred messaging method

 

How This Information Is Used

We use the personal information we collect to communicate with you regarding SHC operations and services, such as:

  • Application for Employment, to help you get signed up for our electronic application service

  • Updates, to keep you informed of important updates on policies and procedures

  • Work related activities, to inform you about scheduling, visits or general work related communications

  • Client communications, to discuss with clients about visit information

 

With Whom This Information Is Shared

We do not sell or rent your personal information. We may, however, share your personal information in limited circumstances, such as with other health care agencies that you have requested to transfer to or from, with your Doctor, or with any healthcare organization directly involved with your healthcare needs. We require our service providers to keep your personal information secure based on HIPAA regulations and privacy, and do not allow them to use or share your personal information for any purpose other than providing services on our behalf.  

 

We may also share your personal information when required by law, or when we believe sharing will help to protect the safety, property, or rights of SHC, employees of SHC, or SHC clients.

 

What Choices You Can Make About Your Information

Text (SMS): If you wish to unsubscribe from text message communications, you can reply with words, such as ‘cancel’, ‘end’, ‘quit’, ‘unsubscribe’, ‘stop’, or ‘stop all’ and you will no longer receive messages.

If you wish to re-subscribe, reply with words, such as ‘start’, ‘yes’, or ‘unstop’.

Please note that unsubscribing from text messages from one campaign will not unsubscribe you from other campaigns unrelated to SHC. Unsubscribing will not remove your information from source SHC systems. 

The accuracy of your contact information you provide us is critical for the delivery of text messages. If you wish to update your contact information, please contact our office at 361-855-9393.

Email: If you wish to unsubscribe from email communications please contact the SHC office staff directly at 361-855-9393. SHC does not use a automated or IT managed emailing service. All emails are directly sent from SHC staff. It is important that you directly contact SHC office staff in order to change your email preferences as it must be dont manually by office staff.
